— Yampa Valley Medical Center and the Northwest Colorado Visiting Nurse Association are teaming up to conduct a community health needs assessment this fall.

The groups are working with HMA Community Strategies to gather information and publish an assessment that will serve as a guide for planning and implementation of healthcare initiatives.

"This unique collaboration between YVMC and VNA reflects our belief in the importance of working together to better understand healthcare needs in Northwest Colorado," YVMC CEO Frank May said in a news release. "This assessment will include a wide range of stakeholders in a formal process to ensure that our strategic focus is to improve the health and well-being of people in our communities."

"This really tells us where we need to focus our efforts in the next three to five years," said Lisa Brown, Northwest Colorado VNA CEO.

Along with primary data solicited from stakeholders, the assessment will dive deeper into specific indicators that measure health and wellness, economic drivers, education, public safety and social environment.
