Board approves purchase of Centennial Home Care

— Home health and hospice services will be added to other health care services already provided by Memorial Regional Health after the board approved the purchase of Centennial Home Care on Thursday.

"We are excited to be combining with a longtime and trusted provider of home health and rehabilitation services in and around Moffat County," stated CEO Andy Daniels in a news release issued moments after the board's approval of the purchase.

MRH expects to expand services by adding both home and in-patient hospice in Moffat County, Daniels stated in the news release.

The new entity will be called Home Health and Hospice at Memorial Regional Health and business offices will be relocated from 487 Yampa Ave. to 785 Russell St. in Craig.

The transition is expected to be complete by June 1 with minimal disruption to current patients and clients.

"We feel that this acquisition will help ensure continuity of care for our patients who often need collaboration between hospital, home and outpatient care services," Daniels stated in the news release.

Kristin Cooper has been named the director of MRH's home health hospice and population management services and will over see the transition.

"I am excited to work with this amazing group of people to offer such valuable services to our community," Cooper stated in the release.
