Feedback forms
Residents who are unable to attend Monday's meeting can fill out a Leadership Profile Assessment, which is available at the Moffat County School District administrative building, 775 Yampa Ave. and at district schools. The form is also available online at www.moffatsd.org under the "District News & Communication" link. Forms may be dropped off at the administrative building or mailed to Hazard, Young, Attea & Associates, Ltd., 1151 Waukegan Road, Glenview, IL, 60025.
Completed forms are due Tuesday.

Community members who want to have a say in selecting Moffat County School District's next superintendent will soon get their wish.
A public meeting is scheduled for 7 p.m. Monday in the boardroom of Moffat County School District administrative building, 775 Yampa Ave. The gathering is designed to collect input about what community members would like to see in the district's next top administrator.
Current Superintendent Pete Bergmann is scheduled to retire at the end of the 2008-09 school year.
Monday's meeting is one in a series intended to collect feedback from "a variety of different stakeholders," said Andrea Camp, Moffat County School Board vice president.
Other meetings with students, staff, administrators and members of parent and school accountability committees are scheduled for Monday and Tuesday. To encourage open dialogue between attendees, these meetings will be closed to the public, Camp said.
Administrators or school board members will not be at Monday's meeting. However, district support staff members may be at the public forum if they are not able to attend their scheduled meeting.
Monday is the only time community members will have to voice their opinions in an open forum. However, feedback forms will be available at the meeting, at district schools and on the district's Web site, www.moffatsd.org.
The form, called a leadership profile assessment, asks residents to list what they think are the district's strengths and weaknesses, as well as characteristics they'd like the next superintendent to exhibit.
It also asks whether respondents know of anyone they think would be a good candidate for the job.
Hazard, Young, Attea & Associates Ltd., the firm hired to help the school district find its next superintendent, recommended the public forum.
"It's a good chance for the board to hear from the community in terms in what's good about the district : and also, issues or concerns and potential challenges" the district faces, said Ellen Bartlett, Hazard, Young, Attea associate.
Feedback from meetings and leadership profile assessments will help guide the firm as it helps the school district search for potential candidates, she added.
Hazard, Young, Attea & Associates will compile feedback from meetings and assessments and present them to the School Board at its Dec. 11 meeting.
Once the school board receives the report, "we'll start putting together exactly what we want from a superintendent," Camp said.
