With cooperation from the local medical community, the athletic physical procedure at Moffat County High School and Craig Middle School has been changed.
The schools have arranged for a $15 voucher to cover physicals that students are required to have to participate in some sports activities.
The vouchers can be picked up at either school.
Once a voucher is obtained, parents can then choose from a list of providers and pay $15 for their child's physical. However, the vouchers will be available during June and July only. After July 31, all athletic physicals will cost the full amount. In addition, the vouchers will only be available at the Moffat County School District Administration Building after June 15.
Craig's medical community has agreed to donate the money collected through the vouchers to the Moffat County High School athletic trainer scholarship fund.
"We give them a $15 voucher and they can get a physical," said Moffat County High School Athletic Director Jim Loughran. "This is a donation by the doctors. The $15 we receive goes into our high school scholarship for trainers. So it's really a positive thing, and this is what the doctors like to do."
To complete the athletic physical voucher process, parents should first pay $15 at Moffat County High School or Craig Middle School. Parents will then receive the voucher and a form required for the physical. A complete immunization record will be required, as will necessary insurance information. All forms must be completed and signed prior to the examination, and all immunization information must be included.
Loughran said the procedure is fairly simple.
"Parents purchase a voucher and pick the clinic of their choice," Loughran said. "They then set up an appointment and take the voucher to that appointment. The doctor will give them a physical and they leave the physical form there and they're done."
Loughran also wanted to remind students and parents the voucher deadline is approaching.
"We're trying to remind the kids that July 31 is the last day that a voucher can be used," Loughran said. "It is important that they understand that the school is not going to have people here after June 15, but they can pick up their vouchers at the administration offices. And I believe that if they go into the main office the secretary will help them, and we'll have a packet ready for them. They can just pick up their voucher and go from there."
Local medical providers include the Moffat County Clinic, the Craig Medical Center, Dr. Craig Rummel and Mountain Medical Specialists. Loughran recommends checking the list of health care providers to be certain a health care provider of choice has agreed to do the physicals.
